Patient Care Pathway
The London Specialised Commissioning Group of the NHS required a visual representation of the possible stages of treatment undertaken by patients suffering from HIV in London. A process of workshops and meetings were organised to research and identify all of the options and outcomes at each stage of a patients care process. The information was then mapped out in a series of diagrams resulting in the flow chart above. The Patient Pathway is now widely used by clinicians both as a reference tool and to inform patients on care processes. The Patient Pathway has been published in NHS reports and publications.
